A Clinical Pharmacy Specialist provides clinical pharmacy services for patients receiving care under the medical direction of a specialty department division or service, regardless of the physical location of the patient within the hospital or ambulatory clinics. They coordinate clinical services with other health care practitioners/educators; supervise the activities of undergraduate and Doctor of Pharmacy students, pharmacy residents, and staff pharmacists assigned to the specialty areas. Responsibilities

Participates in Patient Care Activities.

1. Assess the appropriateness of medications on the basis of health conditions, indication, and therapeutic goals; evaluate the effectiveness, safety, and affordability of medication therapy in respect to patient specific factors such as age, weight, laboratory data and other procedures; identify medication-related problems. Correlate analysis with direct patient observation and findings from the patient's physical exam and communicate analysis with other clinicians and practitioners responsible for the patient's care. 20%

2. The specialist, when functioning in a patient care area, takes responsibility for the continuity of care of a specific patient or team over the entire course of a hospitalization or phase of a disease. The practitioner must ensure that pharmaceutical care is provided when not in the area. This is accomplished through policies and procedures and critical path development. The specialist is responsible for reviewing pharmaceutical care provided during an absence and providing feedback to caregivers when necessary. 10%

3. Provide written and/or verbal consultation related to (but not limited to) intravenous therapy, clinical pharmacokinetics, selection and/or continuance of drug therapy, and determination of therapeutic end points. 15%

4. Provide documentation of all pharmacy related interventions. 5%

5. Provide education and counseling to patients regarding their medication(s) when appropriate. 10%

6. Participate in cardio-pulmonary resuscitation efforts (where applicable). 5%

7. Participate in drug distribution activities during periods of staffing shortage or as scheduled to maintain continuity of departmental services. 10%

8. Provide expert consultation with other health care professionals. Therefore, the practitioner must be thoroughly familiar with the hospital's formulary and all department policies and procedures in order to be a resource person for others. 10%

9. Demonstrate excellent assessment and communication skills on a regular basis. 10%

10. Position requires ability to meet age-specific pharmaceutical needs for all ages (newborn, infant, adolescent, adult, geriatric) with primary focus on adolescent, adult and geriatric population. 5%

Participate in Indirect Patient Care Activities

1. Participate in Multidisciplinary committees/teams to develop clinical pathways and treatment guidelines in order to promote continuous quality improvement and clinical effectiveness. 15%

2. Conduct and participate in drug usage evaluations and continuous quality improvement activities including, but not limited to adverse drug reaction reporting. 10%

3. Exhibit leadership in the participation and development of new clinical services which improve the quality and cost of care provided. 10%

4. Resolve problems relating to drug distribution as identified and/or referred for nurses or decentralized supportive personnel. 10%

5. Participate in the evaluation of drugs and preparation of monographs for the Pharmacy and Therapeutics Committee. 10%

6. Participate in departmental efforts of reviewing Institutional Review Board protocols. 5%

7. Supervise pharmacists, undergraduate students, Doctor of Pharmacy students and pharmacy residents assigned to patient care areas. 10%

8. Maintain and demonstrate competence in clinical problem-solving, judgment, and decision making; communication and education; medical information evaluation and management. Maintain advanced expertise in specialty practice area and recognition locally, regionally or nationally through publications and/or presentations. The specialist should have a network of colleagues in their specialty practice area. 10%

9. Participate in the development of policies, procedures and systems in clinical specialty area. 10%

10. Initiates and contributes to effective cost reduction strategies in the department and hospital. 10%

Participates in Educational Activities.

1. Participate in the education of pharmacy personnel including residents. 40%

2. Participate in the education of non-pharmacy personnel including physicians, nurses, respiratory therapists, and other allied health professionals. 20%

3. Participate in the didactic education of undergraduate students and PharmD. Students. 40%

The practitioner must serve as a role model for all health professionals.

Participates in Research Activities. 5%

1. Demonstrate an ongoing participation in research protocol review/development related to the specialty area. 30%

2. Demonstrate involvement in research related to the specialty area. 30%

3. Assist the Drug Policy Development activities. 20%

4. Serve as a research preceptor for pharmacy students and residents. 20%

Qualifications

KNOWLEDGE AND SKILLS :

EDUCATION:

Doctor of Pharmacy (Pharm.D.) degree or B.S. in Pharmacy with significant clinical experience required; completion of a pharmacy residency and/or fellowship, or equivalent post-licensure experience as a clinical pharmacist is preferred.

YEARS OF EXPERIENCE:

Three years experience in clinical specialty area is preferred in the absence of residency training.

REQUIRED SKILLS AND KNOWLEDGE:

Significant research contributions and drug usage evaluation activity in specialty area of practice

Recognition as an expert in the field of specialty (regionally or nationally) through presentations and/or publication

Demonstrated evidence of exceptional teaching, writing and communication skill

LICENSES & CERTIFICATIONS:

Licensed or eligible for licensure in the State of Ohio
