Pharmacy Resident II - Main Med Surg Pharmacy - Full Time - Days

PGY2 residency programs build upon Doctor of Pharmacy (PharmD) education and PGY1 pharmacy residency training to develop pharmacist practitioners with knowledge, skills, and abilities as defined in the educational competency areas, goals, and objectives for advanced practice areas. Residents who successfully complete PGY2 residency programs are prepared for advanced patient care or other specialized positions, and board certification in the advanced practice area, if available.

Responsibilities

Participate in patient care activities:

1. Facilitates pharmaceutical care services within assigned service/care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards.

2. Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures.

3. Provide written and/or verbal consultation and documentation related to (but not limited to) approved health system policies, therapeutic drug monitoring, selection and/or continuation of drug therapy, and determination of therapeutic end points.

4. Track and document all pharmacy related interventions.

5. Provide education and counseling to patients regarding their medications when appropriate.

6. Participate in medical emergencies (where applicable).

7. Collaborate with and provide expert consultation to other health care professionals. Resident must maintain proficiency in the health system computer systems, formulary, and department policies and procedures.

Participate in drug distribution activities during periods of staffing shortage or as scheduled to maintain continuity of department services.

Participate in indirect patient care activities:

1. Participate in multidisciplinary committees/teams.

2. Draft, review, and revise medication related policies, procedures, guidelines and protocols.

3. Conduct drug usage evaluations and evaluate assigned pharmacy programs for operational, quality, and financial efficiency.

4. Resolve problems related to drug distribution as identified by other healthcare professionals.

5. Participate in the evaluation of drugs and preparation of monographs for the Pharmacy and Therapeutics Committee when needed.

6. Maintain and demonstrate competence in clinical problem-solving, judgment, and decision making. Maintain advanced expertise and network of colleagues in specialty practice area.

Participate in educational activities:

1. Education of pharmacy staff, students, and non-pharmacy personnel including physicians, nurses, and other members of the multidisciplinary care team.

Participate in research activities:

1. Develop and complete a residency research project.

Qualifications

KNOWLEDGE AND SKILLS:

EDUCATION: B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D.) from ACPE accredited college of pharmacy.

Completion of a PGY1 ASHP-accredited or pending accreditation program.
