Registered Nurse - Kidney Transplant - Part Time - Days

Job Description


Demonstrates nursing clinical skills necessary to make pertinent and accurate assessments of the physical, psychosocial, spiritual, and educational needs of the patient.

  1. Demonstrates knowledge base of normal age- specific, physiological, and psychosocial functions of the clinic's population.
  2. Performs a basic physical assessment
  3. Understands signs and symptoms and data related to the patient's presenting problems and potential problems.
  4. Reassess and evaluates patient needs each visit as indicated.
  5. Assesses patient and environmental safety continuously.
  6. Notifies physician, NP and transplant coordinator of pertinent changes in patient condition.
  7. Recognizes and appropriately reports basic abnormal data and subtle changes in patient condition.
  8. Collaborates with other health team members to collect and share pertinent data regarding patient's psychological and physiological condition.
  9. Assesses need for referral to other health care professionals.


Utilizes collected data to establish and prioritize a list of actual and potential, individualized age specific patient problems and/or needs.

  1. Prepares, revises, and coordinates interdisciplinary plan of care for each patient in collaboration with other transplant team members and disciplines.
  2. Includes patient and family in developing goals that are mutually agreed upon.
  3. Assures plan of care is consistent with therapy prescribed by the medical practitioner and standards of care.
  4. Communicates formulated plan of care, as appropriate, to all levels of associates involved in the patients care.
  5. Reviews and modifies plan of care based on changing patient needs.
  6. Demonstrates ability to establish priorities of care.
  7. Updates computerized patient profiles each visit and incorporates changes in plan of care

and status as indicated.


Implements plan of care and interventions to provide quality care based on assessed needs, established standards or care and according to policy and procedures.

  1. Implements nursing actions, either by direct care or delegation of responsibility, based upon patient's plan of care and skill level of personnel.
  2. Implements plan of nursing care in collaboration with the patient, significant others, and other health care team members.
  3. Provides ongoing individualized holistic patient and family education based on needs and plan of care.
  4. Demonstrates critical thinking in review of physician orders and accountability for appropriate implementation of physician orders.
  5. Demonstrates responsibility for medication administration as allowed by licensure and institution policy.
  6. Responds to emergency and other situations requiring immediate attention in a controlled precise and skilled manner.
  7. Makes appropriate referrals to the other health care team members in a timely manner.
  8. Implements safety interventions to maintain patient / family/staff safety.
  9. Reports actual and potential patient safety issues.
  10. Implements measures to prevent contamination and / or transmission of disease
  11. Utilizes appropriate protective devices and equipment to prevent injury.


Continuously evaluates nursing practice in relation to the standards of care and individual plan of care.

  1. Evaluates the effectiveness of nursing interventions.
  2. Involves patient and other health team members in the evaluation process when appropriate.


Nursing process is accurately and concisely documented, including evaluation of treatment.

  1. Documents timely and according to policy and procedure.
  2. Completes forms accurately and completely.
  3. Documents patient / family teaching and response appropriately

Professional Development- Assumes responsibility and accountability for professional growth and development. Contributes to the professional development of peers, colleagues, and others.

  1. Participates in 75% of staff meetings
  2. Supports self and peers in professional growth and development utilizing resources and opportunities within and outside of The Christ Hospital
  3. Completes all educational requirements to maintain competency related to specific population of patients and / or regulatory agencies (Online education 100% on time completion). Submits required tests and paperwork in a timely manner without management intervention.
  4. Participates in self and peer review activities to include the positive recognition of peers and staff on a regular basis
  5. Evaluates and participates in performance improvement initiatives
  6. Participation in community service activities is encouraged
  7. Participation in conferences and educational workshops in relation to current field is encouraged.


Considers factors related to cost and effective outcomes when planning patient care.

  1. Manages workload effectively.
  2. Initiates care in a timely manner.
  3. Abides by the Time and Attendance policy.
  4. Supports effective use of staff and supplies.
  5. Places charges accurately and timely as indicated.
  6. Obtains signed ABN from patient as appropriate and scans into EPIC
  7. Collaborates with and supports management as indicated.


Must be strong skills in phlebotomy and IV insertions.


EDUCATION: Graduate from an accredited School of Nursing. All nurses hired after January 2010 must receive their BSN within 5 years of the date of hire. RN's who were employed at TCH prior to January 2010 and have more than 20 years of experience as an RN are permitted to pursue their BSN on a voluntary basis. An MSN or DNP are not required but superseded the requirement for the BSN.

YEARS OF EXPERIENCE: Minimum of three (3) years in clinical nursing practice. One (1) year experience in ESRD or cardiology or transplant

REQUIRED SKILLS AND KNOWLEDGE: Demonstrates knowledge of nursing, healthcare, and organizational systems and strategies. Demonstrates knowledge and experience in performance improvement theory and application. Knowledge of JCAHO standards and other external organizations related to healthcare evaluation and accreditation. Well-developed communication, facilitation, and interpersonal skills.


Active RN License in State of Ohio / state of employment required, BLS and ACLS required

Membership in Professional Organization preferred, Certification in area of specialty preferred
