Job Description
The primary goals of the Patient Navigator role are to: (1) seamlessly connect patients to the services and resources they need for themselves and those they care for, (2) improve patient experience and outcomes; (3) support, educate, guide and empower the patient / family; (3) bridge gaps in communication and promote timely transition between service providers along the healthcare continuum; (4) remove barriers to care and minimize fragmentation of care for the patient / family; (5) provide a consistent contact person and advocate for the patient / family, in order to improve the experience, satisfaction and to build brand loyalty and (6) promote TCHHN services and resources in the community.
Responsibilities
SPECIFIC DUTIES AND RESPONSIBILITIES
Access Resource
Work in coordination with other service lines/nurse navigators for streamlined access and scheduling. Proactive outreach for process optimization and to build continuity across service lines.
Support additional initiatives:
Leadership -
Assists the Service Line Leadership Team with planning, implementing, and evaluating outcome studies, projects, and research related to work on women's horizontal.
Demonstrates leadership skills such as the ability to motivate, educate, delegate, and mentor peers and other members of the healthcare team while effectively managing the care of patients and family members.
Report outcomes of Patient Navigation program to appropriate leaders and team members, including progress on key metrics:

Understands factors related to cost and effective outcomes when planning and implementing patient care.
FINANCIAL IMPACT AND RESPONSIBILITIES:
This position will track incoming new patients and referrals of existing patients, providing data to be tied back to revenue generation.
Qualifications
EDUCATION: Graduate of an accredited school of nursing. BSN required.
YEARS OF EXPERIENCE: At least 2 years specialty nursing experience is required

Responsibilities

Access Resource
- Direct access to consumers. Positioned as the "expert", available to help guide patients through their healthcare needs via phone or email.
- Provide recommendations on additional services/resource/content TCH needs based on engagement with consumers
- Social media- connect with relevant groups and organizations as an available healthcare resource.
Work in coordination with other service lines/nurse navigators for streamlined access and scheduling. Proactive outreach for process optimization and to build continuity across service lines.
- Work with patients to reduce delays in scheduling services within the network and participate in efforts to streamline ease of patient access
- Community outreach such as providing education at Health Fairs, church groups, etc.
- Uses technologies (e.g., EMR, TCH App, etc.) to enhance communication among patients and health care providers.
- Support navigation needs of Optum Health and maternity COE clients
Support additional initiatives:
- Focus, support and lead efforts through partnerships with Cradle Cincinnati, The Health Collaborative, Queens Village, and Mama Certified to improve healthcare quality and equity.
- Ongoing education and research to stay up to date on the CMS Maternal Opioid Misuse (MOM) model and multi-pronged strategy to ensure TCH approach is in alignment.
- Work in alignment with OBGYN team approach to assist women with comprehensive substance use issues in need of additional support and care navigation.
Leadership -
Assists the Service Line Leadership Team with planning, implementing, and evaluating outcome studies, projects, and research related to work on women's horizontal.
Demonstrates leadership skills such as the ability to motivate, educate, delegate, and mentor peers and other members of the healthcare team while effectively managing the care of patients and family members.
Report outcomes of Patient Navigation program to appropriate leaders and team members, including progress on key metrics:
- Increase in new patients to the network
- Increase in cross promotion of patients across the network (current patients to new services/specialties)
Professional Development -
- Identifies own learning needs based on job expectations

- Attends educational offerings that promote professional development.
- Supports self and peers in professional growth and development utilizing resources and opportunities within and outside of The Christ Hospital. Acts as a resource, educator and preceptor for staff and nursing students.
- Completes all educational, requirements to maintain competency related to specific population of patient and/or regulatory agencies (HealthStream education 100% on-time completion and attendance for annual education day). Submits required tests and paperwork in a timely manner without management interventions.
- Participate in self and peer review activities to include the positive recognition of peers and staff on a regular basis.
- Prepares for certification related to Patient Navigation and/ or specific patient population, when eligible.
- Participates in chosen / relevant professional organizations, conferences and research, as appropriate.
- Evaluates nurse-sensitive outcomes and participates in performance improvement initiatives.
- Participation in research activities is encouraged.

LICENSES & CERTIFICATIONS :
- The RN holds a current licensure to practice nursing in Ohio.
- Membership in professional organizations is desirable.
