Social Worker II-MSW - Bason Place Health and Aging - Full Time - Days

Job Description

Provides psychosocial assessment and knowledge of individual, families, and systems, produces appropriate interventions by using geriatric specific assessment tools such as: Mini-Mental Status Exam (MMSE), Geriatric Depression Scale (GDS), Global Deterioration Scale, and Cornell Scale for Depression in Dementia, and makes appropriate recommendations. Develops and implements a flexible patient focused care and treatment plan in conjunction with a multidisciplinary diagnostic team to effectively and efficiently utilize programs and services as needed. Leads team and trains in utilization of community resources and coordinates referrals within TCH community and outside community, finds resources to meet care needs, identifies any barriers in services, and uses expertise in resource development and training including Advanced Care Planning and pre-need funeral planning, finances, legal considerations and other community resource

Responsibilities

DIRECT SERVICE

  • Evaluates the effects of illness on and assesses psycho-social needs of patients through interviews, chart reviews, and interaction with other health care and community professionals.
  • Develops a plan of treatment that is integrated with the plan of treatment team and appropriate to meet patients' continuum of care needs.
  • Implements the plan of treatment:


-Counsels patients/families to resolve acute crises and mobilizes coping mechanisms.



-Provides linkage to aftercare services to assure continuum of care for patients.



-Facilitates transfer to other facilities, such as ECF's and other hospital



.-Provides psychosocial assessment/knowledge of individual families and systems.



-Develops appropriate interventions by using geriatric specific assessment tools such as: Katz; MMSE; MOCA; Global deterioration scale; Geriatric Depression Scale; clock drawing test

  • Evaluates plan of treatment and makes changes as appropriate.
  • Assumes responsibility for evaluating and meeting own needs for continuing professional education and training to maintain age related, social work, and specialty area competency.
  • Provide referrals for services, identify resources to meet care needs.


Complete advance care planning, legal considerations and other community request



DOCUMENTATION



Provides clear written communication about the psychosocial assessment plan of treatment and the progress of social work intervention. Accountable for timely comprehensive documentation of all professional services.

Professional Development:



• Attendance at social work continuing education opportunities, both local and regional, focusing on health and aging, ethics, and end of life.



• Assure ongoing licensure through the State of Ohio Counselor, Social Worker, and Marriage and Family Therapist Board.



• Procure health and aging specialty certification when eligible and maintain certification status.



INTERDISCIPLINARY TEAM PARTICIPATION

  • Interfaces with other disciplines to enhance quality of care and hospital efficiency.


Attends multidisciplinary patient care rounds, case conferences, and family conferences, as needed.



COMMUNITY NETWORK INTERACTIONS



Maintains positive working relationships with and knowledge about community agencies and services. Identifies barriers in service delivery systems and advocates for change.



CONTRIBUTIONS TO THE DEPARTMENT



Contributes to department by assisting in planning, serving on committees and task forces, and assuming responsibility for delegated projects. Participates in peer supervision, and orientation of new staff. Generates and supports ideas to improve service delivery system.



EDUCATION



Provides education/consultation to students including social workers and other disciplines rotating through the Health and Aging Center

Qualifications

KNOWLEDGE AND SKILLS:



Exceptional interviewing skills, including patients and families in crisis and individuals with a wide range of physical and emotional problems. Ability to prioritize many simultaneous demands. Medical and psychiatric competence including knowledge of disease process and treatment modalities for service area. Knowledge of social health policy and legislation and community resources. Self-awareness, professionalism, and good judgement in dealing with emotionally charge and confidential issues. Excellent verbal and written communication. Skill in social work assessment and treatment modalities necessary to assess and treat individuals, families, and groups. Ability to integrate social work philosophy and ethics into professional practice. This individual functions independently with little supervision. He/she must be able to prioritize many simultaneous demands and meet the needs of patients and families as well as physicians in a timely manner.



EDUCATION: Master of Social Work (MSW)



YEARS OF EXPERIENCE: Two (2) years hospital/clinical setting social work experience
